Summaries of the responsibilities, qualifications and benefits of a volunteer role. Volunteer Application Forms:
Application and Code of Conduct Forms to be completed for either the general volunteer (See: Service descriptions, Section I, above) or Advisory Board Volunteer role. Forms for Club Leaders and Parents of 4-H Members:
Required authorization forms to be completed by parent(s)/guardian in order for a youth to become a 4-H member.
